Palliative Care Nurse, RN Nurse
Trustbridge Hospice, part of Empath Health, is seeking a compassionate and motivated admissions registered nurse (RN) to support patients and families throughout Palm Beach County. This role offers a consistent weekend‑only schedule—every Friday, Saturday, and Sunday from 9:00am to 9:30pm, making it an excellent opportunity for nurses seeking a three‑day workweek, work‑life balance during the week, or an alternative to traditional weekday schedules.

What You'll Do- Serve as the primary nurse responsible for evaluating patients referred for hospice services
- Educate patients and families on hospice care and clearly communicate all components of our continuum of care
- Collaborate with referring physicians and liaisons to coordinate individualized, hospice‑appropriate plans of care
- Act as a key liaison between Trustbridge Hospice and hospitals, physicians, long‑term care facilities, and other healthcare professionals
- Promote and advocate for the hospice philosophy through education and relationship‑building within the healthcare community

- Registered Nurse currently licensed in the State of Florida.
- Current Basic Life Support Certification.
- Minimum of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.
- Understanding of and ability to apply characteristics of Hospice Nursing, case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs
- Basic computer skills and EMR software experience a plus.
- Adequate and reliable means of transportation with adequate insurance and valid Florida driver's license.
