Palliative Care APRN

, is seeking a full-time Palliative Care APRN to join our compassionate, patient-centered team. The APRN will partner with palliative care physicians and an interdisciplinary team to provide comprehensive assessments, symptom management, and care coordination for patients with serious illness. The APRN plays a vital role in supporting patients and families by addressing their physical, emotional, social, and spiritual needs while promoting comfort, quality of life, and goal-centered care throughout their healthcare journey.

• Performs comprehensive health assessments by obtaining medical histories and performing physical examinations
• Diagnoses, treats, and monitors acute and chronic diseases in collaboration with a physician
• Orders, performs, and interprets diagnostic studies in collaboration with a physician
• Identifies implements and evaluates appropriate plans of care, including pharmacologic and non-pharmacologic interventions within the guidelines of the Florida APRN Practice Act
• Promotes positive health behaviors and self-care skills through education and counseling
• Recognizes, reinforces, and participates as an IDT member for palliative care patients by assessing and modifying the plan of care in collaboration with the IDT as necessary to achieve physical, spiritual, social, and psychosocial goals centered on the patient/family goals

• Education:
  
  Master’s Degree in Nursing or a related field
• Licensure:
  Current Florida license as an Advanced Registered Nurse Practitioner. Current DEA (Drug Enforcement Agency) license.
• Certifications:
  
  Valid CPR certification from the American Red Cross or American Heart Association.
• Advanced Certified Hospice and Palliative Nurse (ACHPN) preferred
• NPI provider and ability to bill CMS services or ability to obtain same (if applicable to departmental requirements)
• Minimum of two (2) years nursing experience required.
• Minimum of two (2) year experience as an APRN with experience in palliative care, hospice care, gerontology, or oncology practice preferred
