Responsibilities
This is a Full Time Day shift Case Manager position.
Ensures appropriate utilization of hospital services and develops a discharge plan in a timely manner. Recognizes the signs and symptoms of stroke, initiates the stroke alert, and manages the stroke alert process. Works with the patient and family for discharge planning and treatment based on the patient's needs, condition, and prognosis. Maintains currency in stroke knowledge by completing the annual 1-hour online training.
Roles and Responsibilities- Reports to the Director of Case Management.
- Must be a Florida RN.
- 2-4 years of recent acute care experience required.
- Experience in utilization chart review and discharge planning.
- Managed care experience preferred.
- Knowledge of Inter Qual Criteria.
- Strong clinical background needed with interpersonal communication skills.
- Basic computer skills.
