RN Hospice Field Visits - Weekends

Trustbridge Hospice, a member of Empath Health, is seeking a compassionate and motivated Registered Nurse (RN) to provide rapid response hospice field visits throughout Palm Beach County. What you'll DoThe Field-Based Emergent Care Registered Nurse (RN) provides rapid-response, patient-centered clinical care to individuals in home and community settings. You will:

Be responsible for assessing, triaging, and managing urgent and emergent health conditions outside of traditional clinical environments.

You will operate with a high level of autonomy while collaborating with physicians, advanced practice providers, and interdisciplinary teams to deliver timely, high-quality care and prevent unnecessary emergency department visits or hospitalizations.

Serve as a key advocate for patients and families, ensuring their needs are recognized and addressed

Coordinate comprehensive care under the direction of the primary physician and in collaboration with the interdisciplinary team

Use your clinical expertise to monitor patient conditions, provide interventions, and deliver education that enhances comfort and quality of life

What You'll Need Registered Nurse currently licensed in the State of Florida.

Current Basic Life Support Certification.

One year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing or Hospice nursing preferred.

Understanding of and ability to apply characteristics of Hospice Nursing: case management, crisis intervention, communication skills, teaching skills, thanatology skills and needs

Basic computer skills and EMR software experience a plus.

Adequate and reliable means of transportation with adequate insurance and valid Florida driver’s license.
