Seneca Health Care Center is looking to temporarily add a Subacute Coordinator- LPN to its nursing leadership team. In this role, you'll oversee the daily operations of an assigned nursing unit, ensuring high-quality resident care, staff supervision, regulatory compliance, and exceptional customer service. RESPONSIBILITIES:
* Direct, coordinate, and supervise the activities of nursing staff on an assigned unit
* Assume responsibility and accountability for the overall management of resident care within the unit
* Monitor resident conditions and ensure care plans are developed, maintained, and updated as needed
* Collaborate with physicians, interdisciplinary team members, residents, and families to support quality care outcomes
* Communicate staff concerns and operational issues to appropriate department leaders
* Ensure compliance with facility policies, state and federal regulations, and nursing standards of practice
* Support staff development, coaching, performance management, and team engagement
* Assist with admissions, discharges, family communication, and resident care coordination
* Promote a safe, positive, and resident-centered environment
* Perform additional duties as assigned
REQUIREMENTS:
* Current and valid New York State LPN License required
* Minimum of one year of experience in nursing service administration required
* Additional experience in rehabilitative or geriatric nursing preferred
* Strong knowledge of state, federal, and local long-term care regulations
* Excellent leadership, communication, and organizational skills
* Strong customer service skills and a compassionate approach to resident care
* Must be flexible, dependable, professional, and able to manage multiple priorities
