Family Practice - Without OB Physician

• Schedule: M-F, Full-time
• Types of Cases: Geriatric care for frail elderly population age 55 or older with chronic care needs; conduct history and physical on new participants, event-based and periodic reassessments, develop medical treatment plans, supervise interdisciplinary team care, chronic disease management
• Credentialing: Self-Query NPDB report required at time of presentation (must be pulled within same month as submission). Trinity Health does not reimburse for any application or credentialing fees.

• Board Certified/Board Eligible: Board Certification in Internal Medicine or Family Practice required;
  Board Certification in Geriatrics preferred
• Certifications: DEA Registration required;
  Controlled Substance Registration (CSR/CDS) required
• Licensure: Current and active plenary physician license to practice in the state of Massachusetts required
• Experience: Previous supervisory experience required;
  Minimum of one (1) year of documented experience working with a frail or elderly population required;
  Experience or added qualifications in geriatrics preferred
