We are seeking an experienced and compassionate Medical Director to lead a dynamic PACE (Program of All-Inclusive Care for the Elderly) team in West Springfield, Massachusetts. This is a full-time leadership opportunity to make a meaningful impact in the lives of seniors with chronic care needs while supporting their independence and dignity.
About the Organization
Trinity Health PACE provides comprehensive, community-based care for seniors through the nationally recognized Program of All-Inclusive Care for the Elderly (PACE) model.
Mercy LIFE Massachusetts is dedicated to serving adults 55+ who qualify for nursing home-level care but prefer to remain safely in their homes and communities.
PACE focuses on delivering the entire continuum of care medical, social, emotional, and supportive services all coordinated by an interdisciplinary team.
Schedule
• Monday Friday
• Full-Time
• Participation in on-call rotation as required
️ Position Overview
Reporting to the Executive Director, the Medical Director provides overall medical leadership and oversight of participant care, clinical outcomes, and quality initiatives.
This role includes supervision of Primary Care Providers (PCPs) and Advanced Practice Providers (APPs), oversight of the Quality Assurance Performance Improvement (QAPI) program, and development of medical policies and protocols.
Key Responsibilities
• Conduct comprehensive histories and physicals for new and existing participants
• Develop and oversee individualized treatment plans
• Participate in intake and enrollment evaluations
• Lead interdisciplinary team collaboration and care planning
• Supervise clinical programs and chronic disease management
• Oversee Quality Assurance Performance Improvement (QAPI) initiatives
• Provide education and guidance to families and caregivers
• Support operating goals as a key member of the leadership team
• Manage complex medical needs of frail and elderly populations
Qualifications
• Active Massachusetts physician license (MD/DO)
• Board Certified in Internal Medicine or Family Medicine
• Board Certification in Geriatrics (preferred)
• DEA Registration & Controlled Substance Certification
• Minimum 1 year experience working with frail/elderly populations
• Prior supervisory experience required
• Strong background in chronic disease management
• Self-Query NPDB report required at submission
