Position Summary

This is a full-time, 40-hour per week position. The Family Support Services Supervisor is responsible for coordinating all aspects of patient social services (social work, chaplaincy, & bereavement) as well as volunteer coordination and community outreach. This position is responsible for establishing, implementing, and evaluating goals and objectives to meet and promote the standards of quality and contribute to the PVHPC mission and philosophy.

Job Functions
  • Supervise and coordinate the Family Support Services Department under direct oversight of the Executive Director. The Family Support Services team consists of the Volunteer Coordinator, Bereavement Coordinator, Chaplains, Social Workers, and Integrative Therapists.
  • Develop and implement the objectives, policies, procedures, and job descriptions for the Family Supports Services Department.
  • Ensure that all Family Support Services are delivered consistently with the plan of care and in accordance with standards of practice.
  • Ensure compliance with all state, federal, and CHAP regulatory requirements.
  • Coordinate the implementation of improved work methods and procedures to ensure services are delivered in accordance with policy.
  • Ensure seamless care service delivery by providing oversight of the coordination of care with all clinical service providers.
  • Assist in recruiting, selection, orientation, evaluation, and directly manage members of the Family Support Services Team.
  • Coordinate and supervise the assessment, planning, implementation, and evaluation of the clinical care by members of the Family Support Team.
  • Assure proper documentation in clinical records complies with state, federal, and CHAP standards.
  • In conjunction with the Executive Director, provide leadership in strategic planning, including identifying opportunities for additional or improved services to address unmet customer needs.
  • Maintain a comprehensive working knowledge of the field of social work and share information with appropriate organizational personnel.
  • Maintain knowledge of PVHPC contractual relationships and ensure Family Support Services are delivered according to contractual provisions.
  • Maintain comprehensive knowledge of community resources and assist referral sources in accessing community resources if services not provided by PVHPC.
  • Assist patients and families with practical needs related to or exacerbated by the terminal illness by helping them utilize community resources if needed.
  • In collaboration with the Clinical Educator, plan and implement in-service and continuing education programs to meet the development needs of the Family Support Services Team.
  • Attend and actively participate in weekly IDT meetings.
  • Participate in the PVHPC quality improvement process.
  • Monitor Family Support Services caseload, assign staff to new cases, review documentation for staff training and quality, maintain staff/caseload ratio, and ensure a timely response in meeting referral and client needs.
  • Coordinate and develop community outreach services and presentations.
  • Administrator on-call responsibilities as needed.
  • Maintains a manageable caseload.
  • Other duties as assigned.
Qualifications

(Minimum qualifications will be considered required unless specifically stated otherwise)

Experience
  • Minimum of two (2) years of experience in health care.
  • Experience in hospice care is preferred.
  • Understands hospice philosophy and issues of death/dying.
Education And Training
  • A graduate of a master’s program in social work accredited by the Council on Social Work Education.
License, Certification & Registration
  • Possesses and maintains current CPR Certification.
  • Current Massachusetts licensure: LICSW.
  • Certification in a specialty field, such as family counseling, death and dying, preferred.
  • Valid driver’s license.
Other Requirements
  • Demonstrates good verbal and written communication and organization skills.
  • Ability to deal tactfully with patients, families, and the community.
