Primary Care Registered Nurse

The Center for Human Development (CHD) is expanding its Primary Care services within its Outpatient Behavioral Health Clinic. We are seeking a dedicated Primary Care Registered Nurse to support clients with complex health needs.

• Meet with clients during scheduled visits.
• Conduct nursing assessments and develop care plans.
• Administer medications and educate clients on medication management.
• Review medical records, lab results, and coordinate care with pharmacy.
• Facilitate prior authorizations and manage client triage needs.
• Maintain accurate clinical documentation in the EHR.
• Collaborate with interdisciplinary team members.
• Adapt to a fast‑moving, unpredictable clinical environment.
• Work a Monday‑Friday day shift schedule (8:00 am‑4:00 pm) and travel to other primary‑care locations on an as‑needed basis.

• Registered Nursing (RN) licensure in Massachusetts.
• Minimum of 2 years clinical experience.
• Experience working with patients with serious mental illness.
• Comfortable using electronic health records (EHR).
• Valid BLS CPR certification.
• Reliable, licensed, and insured vehicle for travel between program locations.
• Clean driving record and completed criminal background check.
• Excellent communication skills and ability to adapt to changing needs.
• Self‑motivated and able to thrive in an autonomous environment.

• Salary range: $85,000 – $93,000 (based on experience).
