Licensed Practical Nurse (LPN)

Mentor Adult Day Health, a member of the Sevita family, provides day programs for elders with complex medical or rehabilitative needs. This allows seniors to obtain quality healthcare while remaining active and involved in the community and continuing to live in their own homes. Our programs are staffed with qualified, experienced licensed nurses and caregivers who are trained to provide individual assistance to each person we serve.

What You'll Do

As a Licensed Practical Nurse (LPN), you will play an important role in supporting the health, wellness, and quality of life of program participants by:

  • Assisting with health assessments and participant wellness monitoring
  • Supporting the development and implementation of individualized care plans
  • Maintaining accurate nursing documentation, medication administration records, and physician orders
  • Monitoring participant health status and reporting changes to the appropriate clinical team members
  • Administering medications and treatments in accordance with physician orders and established protocols
  • Promoting health education, wellness initiatives, and preventative care practices
  • Providing support and guidance to program staff regarding participant medical needs
  • Collaborating with Registered Nurses, physicians, specialists, families, and interdisciplinary team members to ensure coordinated care
  • Participating in care planning meetings and contributing to positive participant outcomes
  • Helping maintain a safe, healthy, and person-centered environment for all participants

Qualifications

  • Graduate of an accredited Licensed Practical Nurse (LPN) program
  • Current Massachusetts LPN license required
  • Current CPR and First Aid certification
  • One year of nursing experience preferred, particularly with medically complex adults, seniors, or individuals with disabilities (preferred but not required)
  • Strong communication, organizational, and interpersonal skills
  • Ability to work effectively as part of a multidisciplinary team
