Home Care Position
1-2 yrs home care experience required. Mercy Home Care makes it possible, providing a healthcare team that delivers a wide range of medical services in the comfort of your own home. The services we provide include rehabilitation after surgery or acute illness, such as a stroke; coordinated care for cardiac conditions, including congestive heart failure (CHF); coordinated care for pulmonary (lung) conditions such as COPD;
wound care to aid healing; and palliative care to help provide relief from the symptoms, pain and emotional stress of a chronic or life-limiting condition. Our skilled professionals not only provide medical care and therapies to help you achieve and maintain optimal health, they also assess your progress, stay in touch with your doctor, and provide education and support to you and your caregivers.
Home monitoring helps you manage your health from where you want to be – home. If you have heart disease, diabetes, high blood pressure, or lung disease like COPD, you may need to make lifestyle changes to stay healthy. In addition to following your doctor's instructions about diet, physical activity, and medication, it's also very helpful for your healthcare team to track your vital signs and symptoms on a regular basis.
With home monitoring, your care team can do just that, while you stay at home.
Your home care team works with you, your doctor, family and community agencies to bring you the most complete care possible. For continuity of care, we make every effort to ensure that you have the same team members throughout your recovery. Depending on your individual needs, your home care team may include registered nurses who provide skilled nursing services such as injections, IVs, wound care, patient assessments and education;
physical therapists who help you regain physical function and independence; occupational therapists who help you improve your ability to carry out everyday activities; speech therapists to help restore speech and retrain patients in swallowing, breathing and muscle control; certified home health aides who provide personal care assistance such as bathing, dressing and toileting; medical social workers who assess home situations, assist with finances and facilitate access to community services;
wound specialists who change dressings, administer medications; and wound ostomy care nurses who manage and support patients with a surgical opening for the passage of urine or feces.
