We are seeking a Board-Certified Nurse Practitioner to join a geriatric-focused care team serving an elderly and frail patient population.

Key Responsibilities:

• Perform comprehensive medical assessments
• Develop and manage individualized care plans
• Evaluate and treat acute illnesses
• Participate in discharge planning
• Conduct home visits as clinically indicated
• Coordinate care across the full continuum of services (PACE model)
• Assist with development and review of clinical policies and procedures
• Participate in provider on-call rotation

Submission Requirements:

• Board-Certified Nurse Practitioner (ANCC or AANP) – Required
• Active Massachusetts NP license – Required
• DEA Registration – Required
• CDS Certification – Required
• BLS Certification – Required
• Experience with frail and/or elderly population – Required

Certification Requirements:

• AANP or ANCC

• Controlled Substance Registration (CSR/CDS)

License Requirements:

• Massachusetts Nurse Practitioner License
• NP certification with prescriptive authority
