Physician - Preventive Medicine

We are seeking a dedicated Preventive Medicine Physician for a 60-day assignment with our client in Utah, starting May 25, 2026. This role involves providing essential preventative care to a diverse patient population, with daily shifts from 9:30 AM to 5:30 PM. The physician will focus on chronic care management, complex cases, and reducing hospitalizations through proactive patient contact.

• Provide preventative care to a large homeless population and general super utilizers of healthcare services.
• Make contact with all patients every couple of weeks to ensure continuous care.
• Focus strongly on reducing hospitalizations for patients.
• Manage chronic care conditions such as diabetes and heart disease.
• Handle complex case management and provide comprehensive preventative care.
