Job SummaryCase Managers provide psychosocial rehabilitative services as indicated by the client’s care plan to support clients in meeting individualized goals. They utilize evidence‑based practices, meet fiscal responsibilities, and ensure regulatory compliance and organizational standards are met.
Essential Functions- Provide psychosocial rehabilitative services as described by the Medicaid manual
- Support clients in following individualized care plans and setting case‑management goals
- Complete documentation on services provided according to Valley’s standards
- Meet productivity expectations as assigned by supervisor
- Distribute medications as necessary
- Transport clients to appointments as necessary
- High School diploma or equivalent
- None
- Case Manager certification (post‑hire)
- CPR certification (post‑hire)
- Driving positions require a minimum age of 21 and a current driver’s license
- None
Pay:
Dependent on certification status.
- Uncertified: starts at $17.50. Certification within the first 120 days increases pay.
- Certified: starts at $18.75 and is calculated based on years of certified experience.
Schedule:
Monday – Friday | 8:00 am – 5:00 pm.
- Compensation program includes tenure‑based service increases and performance bonuses.
- On‑Demand Pay gives access to a portion of earned wages before payday.
- Time Off: 15 days of accrued paid time off annually (increasing by one day each year of service), 10 paid holidays, 2 wellness days, and paid parental leave.

Eligible for medical, dental, vision, life, and disability insurance, plus voluntary options. Basic life, accidental death & dismemberment, and long‑term disability insurance at no cost. - Health Savings & Reimbursement:
Up to $2,000 for individuals and $4,000 for families for eligible expenses; HSA contribution with company match up to $900 (individual) or $1,800 (family). - Retirement: 401(k) plan accepts pre‑tax and Roth contributions with a company match up to 6% of salary.
- Professional Development:
