Starting Date: Immediately

• Chair all Student Assistance Team meetings
• Schedule all meetings within 10 days after receiving referral
• Must notify parents of scheduled meeting
• Must notify all personnel involved with student referred (i.e. administrator, teachers, attendance director, transportation director, nurse, etc.)
• Convene Student Assistance Team meeting
• Discuss interventions and record the interventions to be used on Student Assistance Team meeting minutes form
• Set date for review of interventions and progress of student (meeting must be held within one grading period from initial meeting)
• Record review date on Student Assistance Team meeting minutes form
• Record minutes in Student Assistance Team Meeting Log
• Distribute copies of minutes to all invited parties
• Provide parents with copy of minutes of meeting including interventions to be utilized and the date of the review meeting
• Provide parents with Prior Written Notice form if needed
• Send the Student Assistance Team Meeting Log to County Office at the end of the year
• Attend the training for the Student Assistance Team Coordinators at the beginning of each year.
