Job Summary & Requirements

The Palliative Care Clinical Coach is a regionally focused, Ontario Health–funded role responsible for leading the implementation of palliative care priorities across hospital, community, hospice, and regional care settings. The position operates at a program and system level and functions as a clinical implementation lead and project resource, accountable for translating provincial frameworks, funding requirements, and RVH strategic direction into actionable work plans, measurable deliverables, and sustainable improvements in care delivery.

This role is directly aligned with RVH’s Clinical Priorities Plan, specifically the initiative to enhance regional palliative and end-of-life care through the development of a Regional End of Life Care Program, and supports the broader organizational direction reflected in Vitally You. The position plays a key role in operationalizing a phased roadmap focused on system optimization, education and workforce development, integration of models of care, improved care coordination, and long-term program development.

A critical component of this position is contributing to and sustaining a positive, inclusive, and supportive team culture. The Clinical Coach is expected to be a strong culture contributor who actively fosters trust, collaboration, respectful engagement, and shared accountability across teams and project contributors. This includes contributing to a workplace where people feel valued, supported, and empowered to do their best work in service of patients, families, and communities.

The role requires the ability to communicate effectively with a wide range of contributors and audiences, adapting communication style, tone, and level of detail to support relationship‑building, collaboration, and implementation success across clinical teams, leaders, community organizations, and provincial partners.

The position requires a demonstrated commitment to equity‑based, person‑centred, trauma‑informed, and culturally safe care, including the ability to work respectfully and collaboratively with First Nations, Métis, Inuit, and urban Indigenous communities and partners. The role supports care approaches that are guided by community perspectives, cultural understanding, and recognition of the impacts of historical and ongoing inequities, with the goal of improving access, trust, and care experience.

The position is a hybrid role, with the majority of work performed remotely, regular on‑site presence, and occasional travel across the region. The role requires flexibility, adaptability, and the ability to work effectively across both virtual and in‑person environments while maintaining strong relationships and a positive team dynamic.

This position also carries accountability for Ontario Health funding requirements, including development of co‑designed work plans, documentation of activities, data tracking, and reporting to demonstrate achievement of deliverables. All work is focused on implementation, practice change, capacity building, and system integration.

Education:

I. Degree in a regulated health discipline (e.g., RN, NP, RSW or equivalent) required
II. Specialized training in palliative care (e.g., CAPCE or equivalent) required
III. Graduate degree (e.g., MScN, MSW or equivalent) considered an asset

Experience:

  • Minimum 5 years progressive experience in palliative care across hospital and/or community settings
  • 3-5 years of experience in program and project management, including planning, executing, and evaluating initiatives aligned with provincial frameworks and funding requirements.
