Performs patient assessment and plans, delivers and evaluates patient education to selected populations across the continuums of illness and lifespan. Assumes a role as a resource person by participating in community outreach and education support.
Minimum Qualifications Education, Certification, and/Or Licensure
Registration with the Academy of Nutrition & Dietetics.Licensure through the West Virginia Board of Licensed Dietitians, or applicable state licensure where services will be performed.State criminal background check, and federal, if applicable, as required for state department of health (e.g. PA DOH, OH DOH, MD DOH, WV DHHR) regulated areas based on physical work location.Obtain Certification in area of specialty within 1,000 hours of documented diabetes education teaching.Experience
Two (2) year of clinical experience with One (1) year in the care of diabetic patients.Preferred Qualifications Education, Certification, and/Or Licensure
Five (5) year of clinical experience with One (1) year in the care of diabetic patients. Certified Diabetes Care and Education Specialist (CDCES) Certification.

Job Duties and Responsibilities

Identifies education needs of the community and those with diabetes.Performs patient assessment, delivers education and evaluates patient's obtainment of goals.Assists in maintaining the American Diabetes Association certification as applicable.Participates in community outreach and educational support.Advocates on behalf of the patient and families.Communicates with primary care providers, members of the multi-disciplinary team relevant questions concerns, and patient outcomes.Participates in performance improvement activities to assure quality patient care.Completes daily rounds to assess patient status.Ensures all necessary testing and scheduling is complete prior to necessary procedures.Compiles reference materials, summarizes data and prepares reports, and drafts manuals and materials.Provide comprehensive high quality medical nutrition therapy (MNT) specifically to outpatients and in regard to Diabetes education.The Registered and Licensed Dietitians (RD/LD) at WVUH have been granted Order Writing Privileges (with physician co-signature) which have been approved and established by the WVUH Medical Executive Committee and the WVUH Board of Directors. These privileges may be utilized whenever the physician orders the "MNT Protocol". This allows the registered dietitian (RD/LD) to order Medical Nutrition Therapies within their scope of practice as defined by the American Dietetic Association and West Virginia Board of Licensed Dietitians (WBOLD).The RD is considered qualified with current registration from the Commission on Dietetic Registration, current licensure through the WVBOLD, and clinical competency as verified initially and through the annual performance appraisal process. The National Institute for Occupational Safety and Health recommends minimizing the need for employees to manually lift patients and limit safe patient handling to 35 pounds. All transfers, lifts and re-positioning will be completed utilizing the lift equipment and/or other patient handling aids as indicated by the patient's profile and appropriate algorithms.Heavy/Hard work:
