Center Well Home Health is seeking a Home Health Physical Therapist to help patients regain strength, mobility and independence in their homes, reporting to the Clinical Manager. You will deliver personalized care focusing on functional improvement and collaborate with a multidisciplinary team.
The role requires a current Physical Therapy license and at least one year of PT experience. Driver's license and vehicle liability insurance are required as part of Humana’s driver safety program.
