Center Well Home Health is seeking a Medical Social Worker who reports to the Clinical Manager to evaluate and develop care plans addressing the emotional and social needs of home health patients. You will supervise the Social Worker Assistant as part of the patient's plan of care.
You will work within policy and regulatory standards, participate in interdisciplinary care teams, support discharge planning, and help patients access community resources.
