Communicare-Health-Services seeks a Nurse Practitioner for Adult/Gerontology care split between Ivydale and Sutton, WV. Workdays are Monday–Friday, 8a–5p with no on-call, no weekends or holidays. A $15k sign-on bonus is offered for a 1-year commitment, with competitive salary and quarterly productivity bonuses.
The NP will diagnose, treat, and manage chronic and acute conditions, educate residents, and collaborate with PCPs and the care team in a high-touch, value-based care model.
