Center Well Home Health, in partnership with Humana Inc., is hiring a Home Health Physical Therapist to deliver in-home therapy, helping patients regain strength, mobility, and independence within their own homes.
You will be part of a clinical team, reporting to the Clinical Manager, and will assess progress, document care, and coordinate with physicians to meet patient goals. A 40-hour work week and a $20,000 sign-on bonus are included.
