Therapist – Physical Therapist

$2502.40/week –

Job Summary:

Seeking a licensed Physical Therapist to provide comprehensive outpatient and home health therapy services. The therapist will assess, plan, and implement patient rehabilitation programs to promote mobility and functional independence.

Responsibilities:

• Evaluate patients’ physical conditions and develop personalized treatment plans.
• Administer therapeutic interventions, including orthopedics, weakness management, and cardiac rehab.
• Document patient progress and adjust treatment strategies accordingly.
• Collaborate with healthcare team members to ensure continuity of care.
• Educate patients and families on exercises, injury prevention, and health maintenance.
• Maintain compliance with all licensing and certification requirements.

Qualifications:

• Valid state license as a Physical Therapist.
• BLS certification required; relevant state certifications necessary.
• Experience in home health, outpatient, or inpatient settings preferred.
• Ability to work independently and adapt to different county locations.
• Strong communication and documentation skills, including familiarity with electronic health records.
• Positive attitude with willingness to learn and adapt to Epic documentation system.
