The Supervisor Community Health collaborates with the Manager of Preventive Medicine Department and Preventive Medicine Nurse Navigator to supervise Community Healthcare Workers' scheduling, payroll, community programs, and preventive medicine assignments, and data collection. Assists in monitoring performance improvement and hospital wide requirements for employees, patient care and safety.

* MINIMUM QUALIFICATIONS:
**** EDUCATION, CERTIFICATION, AND/OR LICENSURE:
** 1. High School diploma or equivalent.
2. Valid Driver’s License. 3. CPR Certification.

* EXPERIENCE:
** 1. Three (3) years of experience as a Community Healthcare Worker or in a healthcare setting.
** To be completed as established by department: (Administered at Department level)
** 1. Community Healthcare Workers or Community Health Education Resources Person Level 2

** 1. Some vocation or college courses completed.

DUTIES AND RESPONSIBILITIES:

1. Supervises Community Health Worker staff including scheduling, payroll, community programs and preventative medicine assignments and data collection.
2. Assists with monitoring performance improvement and hospital wide requirements for employees, patient care and safety.
3. Assist clients in assessing health related services, including but not limited to: obtaining a medical home, providing instruction on appropriate use of Medical Home, overcoming barriers to obtaining needed medical care and social services.
4. Coaches patients in effective management of their chronic health conditions and self-care.
5. Helps patients design and keep a personal health record, develop health management plans and goals.
6. Helps community members understand the importance of health screenings, immunizations, and, routine check- ups.
7. Assist community members on how to shop healthy and wisely and help them with finding needed resources as identified.
8. Serve as a link and works collaboratively with health care professionals ,social services, and the patient/community.
9. Takes part in follow up with health management care plans with both patients and providers.
10. May at times assist with scheduling and insurance pre-authorizations for services . Continuously expands knowledge and understanding of community resources, operations, functions, and resources available to handle new or unusual situations.
12. Effectively works with people from diverse backgrounds in reducing cultural and socioeconomic barriers between clients and institutions.
13. Establishes positive supportive relationships with participants and provider’s feedback.
14. Helps clients in utilizing resources, including scheduling appointments and assisting with completion of applications for programs for which they may be eligible.
