Sevita - - Responsibilities:
Provide direct support or assistance in accordance with individual service or program plans; assist with socialization and behavioral development;
Assist with daily activities: personal care, housekeeping, recreational activities, transportation, community orientation, shopping, and activities of daily living;
Accurately document progress and activity; review records and logs to stay abreast of changes in service plans; maintain confidentiality;
Monitor individual health concerns; communicate with nurse or supervisor; call 911 in emergencies; follow health care directives;
Transport individuals as needed and maintain safety of equipment and environmental conditions
