The Medical Assistant is responsible to the Director of Nursing for rendering nursing care in assigned areas in terms of individualized patient needs, utilizing nursing techniques, procedures, and established standards based on their education and training. Perform routine administrative and clinical tasks. Provide quality care to patients by supporting CCWV quality initiatives. Assist the patient in navigating the health care system.

Responsibilities

and Essential Duties
  • Escort patients to the exam room and initiate sensitive conversations. This includes but is not limited to verifying demographic information, emergency contacts, next of kin, preferred pharmacy, HIPAA Notice of Privacy Practices, family size and income level, social security number, medical release forms, etc. and documenting in the correct fields within the patient’s electronic health record. Make sure that Primary Department and Usual Provider-Primary Care Provider matches.
  • Ensure that registration paperwork is completed annually. Assist patients with electronic check-in when necessary.
  • Obtain patient medical history and record vital signs (height, pulse, temperature, visual acuity, weight, and measures blood pressure, head circumference where indicated for patients). The MA will ask and document responses to patient health, social history questions, and appropriate screenings during the intake process.
  • Register the patient for the portal, if applicable. Provide education for use of the system, highlight communication options, and help create an initial passcode. Troubleshoot problems.
  • During intake, evaluate responses to social determinants of health questions. Provide community resources packet when needed. Report high-risk responses to care team.
  • Pre-visit planning:
    Call patients before appointment to review status, request consult notes, remind about outstanding orders, and ensure insurance eligibility. Confirm appointment type and adjust if needed. Schedule next visit prior to patient leaving.
  • Follow up on patient reminders, lab and imaging orders, specialist referrals, and results, ensuring they tie to quality measures and outstanding orders.
  • Annotate charts with exam and treatment results. Contact patients with laboratory and specialist findings. Document outreach.
  • Daily equipment checks and sanitary duties. Clean and sterilize instruments and exam rooms, restock supplies.
  • Utilize MA program knowledge to safely provide primary preventive and therapeutic care. Perform delegated independent tasks per CCWV procedures.
  • Contribute to development of patient/caregiver plan of care. Carry out physician orders accurately.
  • Administer medication accurately following right patient, right medication, right dose, right route, right time, and documentation. Observe patient response.
  • Perform venous blood draws.
  • Maintain and coordinate communication among patient/caregiver, staff, and provider. Report patient needs to physician.
  • Work collaboratively with other health care team members.
  • Provide patient health teaching as delegated. Use written materials and appropriate teaching methods. Document teaching.
  • Attend staff development programs and meetings.
  • Monitor medication/vaccination inventory and notify Purchasing when low.
  • Counsel patients on medication refills and questions, and direct calls appropriately.
  • Perform federal urine drug screenings after training.
  • May serve as medical receptionist when needed.
  • Other duties as assigned.
