• Conduct comprehensive physical therapy evaluations and develop individualized treatment plans for residents in a SNF setting.
• Implement and progress therapeutic interventions to improve mobility, strength, balance, gait, and functional independence.
• Provide gait training, neuromuscular re-education, therapeutic exercise, and manual therapy as appropriate.
• Document assessments, treatment plans, progress notes, and outcomes; communicate progress with the interdisciplinary team.
• Ensure patient safety during transfers and daily activities; select and apply appropriate equipment and assistive devices.
• Collaborate with nursing, therapy, and social work teams to coordinate care, discharge planning, and caregiver education.

• Active Physical Therapist license (state-specific) or eligibility for licensure as required for the assignment.
• Ability to work the listed shift (08:00–16:30) in Shepherdstown, WV for 17 weeks.
• Willingness to travel to assignment location; travel-ready as part of a travel assignment.
• Experience related to physical therapy practice; prior skilled nursing facility experience may be preferred.
• Strong documentation, communication, and interdisciplinary collaboration skills.
• Demonstrated ability to work in a resident-focused, rehabilitation-oriented setting.
