Location: Cowen
Home Care Plus Inc, part of United Health Group, is seeking a Physical Therapist for Home Health to assess and treat patients in Greenbrier, Pocahontas and Monroe Counties. You will evaluate functional status, mobility, endurance, pain, and other conditions, and implement a care plan while monitoring progress with the care team.
You will collaborate with physicians and nurses, educate patients and families, document visits, and ensure care aligns with state guidelines.
