Case Manager/Care Coordinator Level 2 – Outpatient Services

Location: McDowell County, West Virginia
Employment Type: Full-Time

Position Summary

Southern Highlands Community Mental Health Center is seeking a compassionate, dependable, and organized Case Manager/Care Coordinator Level 2 to join our Outpatient Services team in McDowell County.

This position assists clients and families in accessing the services and resources needed to promote stability, independence, and overall well-being. The Case Manager/Care Coordinator works closely with clients, families, treatment providers, and community organizations to coordinate care, advocate for client needs, and support progress toward individualized treatment goals.

Education and Licensure Requirements

Applicants must meet at least one of the following qualification pathways:

  • Bachelor’s or master’s degree in a human-services-related field that meets the requirements established by the West Virginia Board of Social Work;
  • Licensed psychologist with a master’s or doctoral degree from an accredited program; or
  • Licensed registered nurse.

  • Strong communication, organization, documentation, and time-management skills.
  • Ability to establish professional and supportive relationships with clients and families.
  • Ability to collaborate effectively with treatment providers and community partners.
  • Knowledge of behavioral health services and community resources.
  • Ability to manage multiple priorities and respond appropriately to changing client needs.
  • Commitment to providing respectful, compassionate, and person-centered services.
  • Ability to maintain professional boundaries and protect confidential information.
