This is a great option for an experienced PT who enjoys the autonomy and one-on-one patient care of Home Health
, while working with patients in their own homes to improve mobility, safety and independence.

• Home Health setting
• 1:1 patient care
• Adult and geriatric population
• Perform evaluations and develop individualized plans of care
• Gait, balance and transfer training
• Strengthening and functional mobility
• Fall prevention and home-safety education
• Patient and caregiver education
• Help patients regain independence within their home environment
• Coordinate care with physicians, nursing and other members of the interdisciplinary team
