Here's What You'll Do for Our Residents & Facility:

Supervises Staff:
Manages, hires, trains, and schedules social work staff and interns.
Administrative:
Establishes departmental policies, procedures, and quality improvement standards & oversees the social services department budget and resource allocation.

Enforces Regulations:
Ensures the facility complies with federal (CMS), state, and local social service mandates & Serves as the primary department contact during state health inspections and surveys
Maintains Documentation:
Maintains & audits medical charts to ensure psychosocial documentation meets strict legal standards.

Collaboration:

Participates in care planning and other meetings, including Quality Assurance and Performance Improvement (QAPI).
Care & Mediation:
Steps in resolving severe, complex family grievances or resident crises. Provides supportive care as needed to supplement social work needs
Community Liaison:
Builds relationships with local hospitals, mental health organizations, and community resources to streamline admissions and discharges.

Does This Sound Like You?

Licensed to work within the State of Connecticut as a LSW Licensed Social Worker
Able to pass background check, drug screen, professional references check & physical evaluation
High level understanding of leadership, customer service and clinical competency
Leadership experience in a long-term care setting
Prior experience as a Director of Social Services/Social Work Director
