ACT Mental Health Counselor

This position is located at the 3 Mile location of Mountain Laurel Integrated Healthcare in Logan, WV.

• Provide direct services to individuals, families, and groups.
• To work collaboratively with the ACT physician and other providers in meeting the health and behavioral health needs of the ACT consumer.
• Provides crisis intervention services in office and in the community. Provides emergency on call phone support to ACT consumers and team members as needed.
• Participate in daily organizational meetings.
• Participate in treatment planning as leader. Maintain correct and proper diagnosis, and aid in social history updates.
• Be a part of crisis through telephone and VTC modalities.
• Assists in the overall team case management, counseling, and support functions.
• To provide in-home, community, and transitional care supports/interventions to patients involved in project as needed.
• To maintain accurate and timely information in the electronic health record (EHR) and support the mission of meaningful use measures.
• To serve as a consultant to team on health and safety concerns.
• Performs other related duties and responsibilities as required.
• Will work non-traditional hours.

Graduation from an accredited college or university with A Master’s Degree in the field of social work, counseling or psychology and experience in behavioral health services and substance abuse assessment/treatment.

Must have a valid West Virginia driver’s license with a good driving record. Ability to function as a team member with good organizational skills. This position requires no history of ethical violations substantiated by a West Virginia licensing board.
