Behavioral Health Therapist
Camden Family Health, a Federally Qualified Health Center (FQHC), is seeking an experienced and mission-driven Behavioral Health Therapist for our Camden On Gauley, and Richwood locations.
Description of Duties:
- Performs psychosocial assessments and discharge planning for patients.
- Participates in patient care planning responsive to age specific needs.
- Familiarity with medical and insurance legislation and insurance authorizations
- Provides behavioral health therapy to needed populations
- Refers patients for inpatient treatment as needed
- Provides discharge services and active follow-up to engage the client in appropriate services
- Attends treatment team meetings, and maintains contact with referral sources (e.g., Probation, DSS, IEP's in school settings, etc.) as needed.
- Ability to function in the Electronic Medical Records by completing documents all assessments, treatment sessions, contacts, case management and other work done on behalf of patients according to guidelines.
- Exhibits willingness to work under the direction of a program director/Lead therapist
- Participates in regular treatment team meetings to discuss clinical cases, review and update patient treatment plans, monitor therapeutic progress and initiate recommendations for referral and continuing care
- Completes incident and error forms when indicated
- Reviews cases and/or groups periodically, as well as serious clinical patient condition changes with supervisor and other clinical personnel as indicated.
- Under direct supervision, conducts assessments of new patients using the identified format for documentation and clinical decisions regarding treatment, interventions and referrals, as instructed. Implements, improves, and adapts concepts, models, and constructs of treatment for application in service delivery to patients.
- Completes annual training for Care Learning
- Follows HIPPA guidelines and 42 CFR to ensure the patient's privacy and confidentiality
- Participates as a member of the patient Care Team by: using the CFH Care Team Communication Process for receiving, sending and documenting all patient related information and tasks with other Care Team members.
- All other duties as assigned
Qualifications:
- Must be a Master Social Workers, Licensed Professional Counselor, or a Licensed, Clinical Psychologist
- Must exemplify compassion, communications skills, active-listening skills, patience, and interpersonal skills
- Knowledge of healthcare operations
- Familiarity with medical and insurance legislation
- Excellent organizational and multitasking skills
- Must maintain CEU's requirements for licensing
- Will need to be available for crisis intervention and on call hours as needed
- Must provide a valid driver's license and up to date insurance
- Knowledge of psychiatric evaluation and assessment process, as well as application of current version of the DSM preferred.
- Experience in diagnosing and treating behavioral and psychiatric conditions preferred.
