Overview
The home health nurse provides direct patient care in the patient's home environment as part of a multidisciplinary team including PT, OT, speech, social work and home health techs. The nurse evaluates patients and uses professional judgment to design the individualized plan of care which best meets the patient's needs. The nurse provides direct, one-on-one patient care and supervises care provided by aides.
The home health agency relies heavily on technology; email is a primary means of communication and the nurse uses a laptop to complete documentation in an electronic record. The patient population varies across acuity, age, and diagnosis. The nurse works under the direction of a clinical team manager. Employment begins with a 6- to 10-week orientation with a seasoned preceptor to ensure preparation for independent treatment.
Additional job duties include performance improvement opportunities, policy and procedure review and development, research, etc. Working in home health offers variety and real-life clinical challenges and the opportunity to impact patients in their daily lives.
- Provide direct patient care in the home as part of a multidisciplinary team.
- Evaluate patients and design individualized plans of care that meet patient needs.
- Supervise care provided by aides.
- Use a laptop to complete documentation in an electronic record; primarily communicate via email.
- Work under the direction of a clinical team manager.
- Participate in orientation, training, and ongoing professional development.
- Engage in performance improvement opportunities, policy and procedure review and development, and research.
- Contribute to the variety and clinical challenges inherent in home health care.
- Previous home health experience preferred, but not required.
- Must meet all requirements of an LPN I ladder to advance to LPN II.
- LPN Nursing License required.
- BLS Certification (Basic Life Support) – American Heart Association Healthcare Provider (HCP) required.
- Current driver’s license and reliable transportation.
- Must be licensed or eligible in the State of West Virginia; licensure in Virginia may be required depending on geographic area.
- New hires must have American Heart Association certification prior to completion of orientation.
- Ability to transfer computer data and maintain reliable means of phone communication with hospital and home health.
