ROANE GENERAL HOSPITAL

Roane General Hospital in Spencer, WV a 60 bed Critical Access Hospital with 35 SNF/ICF beds, 25 acute care beds, Level IV Trauma Emergency Department, three Rural Health Clinic and walk‑in clinic is currently seeking applicants for Full‑Time RN Nurse Navigator for Infusion Services/Oncology.

POSITION SUMMARY

The Oncology/ Infusion Navigator will be responsible for managing the care of identified patients with cancer, hematologic diseases and other disease processes in collaboration with a multidisciplinary team to ensure high quality care across all care delivery sites. Serves as the main point of contact for patients, caregivers, and referring physicians; provides resources and assistance with accessing clinical and supportive care services offered within Roane General Hospital and the surrounding community.

The Oncology/ Infusion Navigator demonstrates critical thinking and uses the nursing process to assess and intervene to meet the needs of patients and their families/caregivers by providing care coordination throughout the cancer continuum to improve health, treatment, or end‑of‑life outcomes.

QUALIFICATIONS

Education and/or Experience Minimum
  • Current West Virginia license as a Registered Nurse (RN)
  • Associate’s degree from a college or university required
  • Three years of clinical nursing experience
  • Strong assessment and intravenous access skills
  • Strong oral and written communication skills
  • Advanced computer skills
Preferred
  • Bachelor’s Degree in Nursing
  • Previous oncology experience
  • Previous nursing case management experience
Certifications, Licenses, Registrations

Must be obtained within 30 days of hire:

  • Current certification in Basic Cardiac Life Support (CPR) for providers
  • ONCC Chemotherapy/Biotherapy Certification and start oncology orientation (maybe not be at Roane General’s campus, travel may be required)

Must be obtained within 6 months of hire:

  • Current certification in Advanced Cardiac Life Support (ACLS)
  • Current certification in Pediatric Advanced Life Support (PALS) or equivalent certification

Must be obtained within 2 years of hire:

  • Oncology Certified Nurse (OCN)
  • Blood and Marrow Transplant Nurse Certification (BMTCN)

Optional certifications:

  • Care Coordination and Transition Management (CCTM)
Core Duties and Responsibilities

This position will be responsible for all aspects of patient care specific to the Oncology/Infusion Therapy Department.

  • Ability to communicate clearly and effectively with varied persons including patients, families, physicians, other clinical and non‑clinical staff, and administrators.
  • Assesses patient needs upon initial encounter and periodically throughout navigation, matching unmet needs with appropriate services and referrals and support services, such as palliative care, dietitians, medical providers, social work, pre/rehabilitation, and legal and financial services.
  • Identifies potential and realized barriers to care (e.g., transportation, child care, elder care, housing, language, culture, literacy, role disparity, psychosocial, employment, financial, insurance) and facilitates referrals as appropriate to mitigate barriers.
  • Develops knowledge of available local, community, or national resources and the quality of services provided; also establishes and maintains professional relationships with the providers of these services.
