This Home Health Registered Nurse position is based in Anmoore, West Virginia, a small, welcoming community in the Clarksburg area of North-Central West Virginia. In this role, the Home Health RN delivers one-on-one, patient-centered care in the home setting throughout Anmoore and surrounding communities. You will work as an integral member of a collaborative, interdisciplinary home health team that may include physical, occupational, and speech therapists, medical social workers, and home health aides. The position is well suited for nurses who value autonomy, enjoy building long-term relationships with patients and families, and appreciate seeing the direct impact of their care in a home environment.
Typical responsibilities include performing comprehensive patient assessments in the home, developing and updating individualized Plans of Care, managing medications, providing disease-specific and preventive education, coordinating care with physicians and other disciplines, and monitoring for changes in condition that may require intervention. You will support patients with a variety of diagnoses, such as heart failure, COPD, diabetes, post-surgical recovery, and other acute and chronic conditions, with a focus on promoting independence and preventing avoidable hospitalizations.
A typical day in this position involves driving to multiple patient homes within a defined geographic territory, completing scheduled visits, documenting care in the EMR, and communicating regularly with your clinical manager and team. Visit volume is structured to allow time for thorough assessments, patient teaching, and safe, accurate documentation. You can expect to work with a mix of new admissions, routine follow-up visits, and discharge planning as patients meet their goals.
Shifts are generally daytime hours with a weekday schedule and occasional rotation for after-hours or weekend coverage depending on agency needs. The structure provides a degree of scheduling predictability while still offering variety in your day-to-day work. Patient caseload and visit expectations are designed to support safe, high-quality care without sacrificing the meaningful connections that make home health nursing rewarding.
