ResponsibilitiesDuring a visit that can last up to one hour, providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. The Matrix Comprehensive Health Assessment (CHA) captures insight that is difficult to obtain in routine office visits and helps close multiple care gaps with a single visit.
Key duties include:
- Conduct adult/geriatric assessments, including medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation per Matrix guidelines.
- Administer and order point‑of‑care and lab‑based testing as needed.
- Collaborate with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
- Work with the Primary Care Physician (PCP) on patient education and follow‑up as necessary.
- Provide services at the member’s place of residence, whether in their home or a skilled nursing facility.
- Flexibility – Visits are scheduled based on your availability. We conduct assessments Sunday‑Saturday from 8 am to 7 pm, allowing you to work during the day, evening, or weekend.
- Competitive Pay – Earn income based on the number of assessments you complete. Those completing an average of 20–100 monthly visits may earn between $1,800 and $11,500 per month, depending on the per‑assessment rate of $90–$115 and your state of residence.
- Benefits – Mileage reimbursement, 401(k), employee referral bonuses, and limited voluntary benefits.
- Education and Support – Comprehensive onboarding training, a designated preceptor, 1:1 feedback, and free access to continuing education.
- Additional Licensure – We support obtaining additional state licensure and credentialing in neighboring states when business needs arise.
- Full‑time Opportunities – PRN providers can transition to full‑time roles supporting their home location, a state, or region, and receive a full‑time sign‑on bonus.
- Insurance Coverage – Bodily Injury: $100,000 per person/$300,000 per incident;
Property Damage: $50,000;
Collision and comprehensive deductibles: $1,000 or less.
- Master’s degree or commensurate experience and satisfactory completion of NP licensure.
- Current NP licensure in state of practice, including prescription authority or ability to obtain it.
- AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
- Current BLS, ACLS, or CPR certification.
- 1 year of NP experience preferred; new graduates encouraged to apply.
- Valid state driver’s license for the state of residence and automobile insurance coverage required (or ability to obtain).
- Strong computer skills and familiarity with employee health/medical record software.
- Excellent verbal and written communication skills.
