Position: Transplant Coordinator I
Location: Evansdale

Transplant Coordinator I oversees the plans, directs and provides specialized nursing care with the primary emphasis on the treatment of individuals with end stage organ disease that are designated to have a solid organ transplant. Provides continuity and coordination of patient care, conducts patient and professional education initiatives. They will collaborate relationships with the multidisciplinary teams, community resources and agencies. Actively participates in the WVU Medicine organ donor awareness activities.

Minimum Qualifications

Education, Certification, and/or Licensure:

  • Current Registered Nurse license issued by the state in which services will be provided or current multi‑state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).
  • Obtain certification in Basic Life Support within 30 days of hire date.
Experience
  • Three (3) years nursing clinical experience.

Core Duties and Responsibilities
  • Transplant Referral: Identifies appropriate candidate upon referral and reviews with appropriate staff. Reviews records and identifies needs for the evaluation – phone evaluation. Works with the Financial Coordinator on insurance authorization to proceed.
  • Transplant Evaluation: Coordinates the evaluation process with the transplant team members following established protocols for testing.
  • Advocates for the patient utilizing the information learned during discussions with patient/family. Participates in Patient Selection Committee.
  • Provide education on transplant evaluation, OPTN regulations, organ allocation, waiting phase, transplant process, risks/benefits, post‑operative phase, immunosuppression and long‑term follow‑up. Provide education on utilization of expanded donors and high‑risk donors.
  • Track testing to secure all needed consults/testing have been completed and review findings. Complies with OPTN/UNOS policies and listing procedures and communications to patient/providers.
  • Continue with educational plan based upon the patients learning needs. Keep an ongoing line of communication with patient and referring MD to follow patient status and needs for transplant team involvement.
  • Assess patient for any changes to medical condition and address with the Transplant Team (fitness to remain active on list or to undergo transplant).
  • Follows protocols regarding testing frequency during the waiting phase. Update and maintain the OPTN/UNOS listing requirements as applicable and update Team on any changes. If candidate dies while waiting, remove from OPTN/UNOS listing according to protocol.
  • When offer is available, follow the WVU Protocol. Assess patient's current condition to determine if eligible for transplant and communicate per protocol. Facilitate admission/transfer to WVU. Follow OPTN/UNOS guidelines regarding patient removal from the list. Notify OPO of transplant outcome and patient info for Donor Family. Secures that Referring MD/PCP is notified of the transplant.
  • Monitor patient progress. Educate patient/family/support regarding care and responsibilities after discharge. Work with Multidisciplinary Transplant Team to develop a discharge plan of care. Provide OPO with recipient follow‑up. Communicate with Referring MD/PCP regarding discharge status.
  • Work with patient/family/support to achieve optimal physical, social and emotional care. Collaborate with Multidisciplinary Transplant Team. Reinforce and continue with post‑transplant education.
  • Secure testing being done/received according to the schedule. Review all testing coming in on patient and act according to protocol. Implement changes to treatment plan per MD/protocol.
  • Schedule any needed follow‑up testing/consults/hospitalizations/etc. Monitor patients compliance with medication and medical plan. Reinforce need for well care (dentist, mammograms, colonoscopy, etc). Assess needs for immunizations.
  • Attend post‑Transplant meetings to discuss any issues/concerns.
