ResponsibilitiesAs a provider, you will conduct in-home or skilled nursing facility assessments lasting up to one hour.
Responsibilities include:
- Conduct adult or geriatric assessments covering medical history, diagnosis, treatment, health education, and referrals.
- Administer and order point‑of‑care and laboratory testing as needed.
- Collaborate with physicians, case managers, social workers, family members, and other medical personnel.
- Coordinate with the Primary Care Physician on patient education and follow‑up.
- Provide comprehensive documentation following Matrix guidelines and protocols.
- Flexibility – Schedule visits Sunday through Saturday, 8 am–7 pm, based on your availability.
- Competitive Pay – Earn income per completed assessment; potential earnings range from $1,800 to $11,500 per month, depending on visit volume and state rates.

- Education and Support – Comprehensive onboarding, a preceptor, 1:1 feedback, and free continuing‑education access.
- Additional Licensure Support – Assistance obtaining licensure and credentialing in neighboring states if needed.
- Transition to Full‑time – PRN providers can move to full‑time roles with a sign‑on bonus.
- Sign‑On Bonus – $2,000 sign‑on bonus for qualified applicants.
- Master’s degree or equivalent experience and successful completion of NP licensure.
- Current NP licensure in the state of practice, including prescription authority or ability to obtain it.
- Board certification in one of the following: AANP, AACN, or ANCC for Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialties.
- Current BLS, ACLS, or CPR certification.
- Minimum 1 year of NP experience preferred; new graduates encouraged.
- Valid driver’s license in the state of residence and required automobile insurance coverage.
- Insurance limits:
Bodily Injury $100,000 per person / $300,000 per incident;
Property Damage $50,000;
Collision/Comprehensive deductibles $1,000 or less. - Strong computer skills and familiarity with employee health and medical record software.
- Excellent verbal and written communication skills.
