About the Role

Align Med Partners, a healthcare management company located in Addison, TX, is seeking a Nurse Practitioner to join its Healthcare & Medical Services team. In this role, you will provide high-quality, patient-centered care within a collaborative, innovation-driven environment that supports both clinical excellence and professional growth. As a Nurse Practitioner with Align Med Partners, you will conduct comprehensive patient assessments, obtain detailed histories, perform physical examinations, and formulate differential diagnoses for a diverse patient population.

You will develop and implement evidence-based treatment plans, order and interpret diagnostic tests, prescribe medications and therapies in accordance with state and federal regulations, and provide ongoing management for both acute and chronic conditions.

• Coordinate care across settings with physicians, nurses, and other interdisciplinary team members to ensure continuity, safety, and optimal patient outcomes.
• Educate patients and families on diagnoses, treatment options, medication management, and preventive health strategies, promoting engagement and self-management.
• Maintain thorough, accurate documentation in the Electronic Health Record (EHR), adhere to regulatory and compliance standards, and participate in quality improvement initiatives to enhance clinical processes and patient satisfaction.
