Sub-Acute, Long-Term Care offers a rewarding and flexible work environment with no on‑call or weekend duty. We are seeking a full‑time (40 hours per week) Advanced Practice Provider—Nurse Practitioner (NP) or Physician Assistant (PA)—for our Cedar Ridge Center in Sissonville, WV. The base salary ranges from $104,000 to $120,000 annually, plus a no‑cap incentive compensation bonus program.
Responsibilities- Report directly to the Director of Clinical Practice and/or Area Clinical Practice Manager.
- Collaborate on‑site with the Center Leadership Team to achieve center‑specific goals and priorities.
- Work with the Medical Director and/or Attending Physician to provide patient‑specific medical care.
- Deliver primary and urgent care services to nursing‑center residents in partnership with other medical providers.
- Assist Center Leadership in delivering high‑quality health care and everyday compassion in line with Align Med Partners mission.
- Graduate of an accredited School of Nursing; licensed registered nurse.
- Certified Nurse Practitioner with a Master’s Degree in Nursing and a concentration in Older Adult and/or Family Nurse Practitioner (FNP). Other concentrations may be considered based on experience and business need.
- Physician Assistant graduates of a program accredited by the ARC‑PA, passed PA National Certifying Exam (PANCE), with PA state licensure and NCCPA certification.
- Two years of experience working with adult or geriatric populations is preferred.
- In states where practice is allowed, an unrestricted professional license and DEA for APRN and PA in each state of practice.
- Comprehensive 90‑day training program and continued support.
