Schedule:

• Provide 1:1 care following the client’s plan of care at all times while maintaining dignity and independence
• Administer medications, feedings, oxygen, ostomy care as needed per the client’s plan and assessments
• Accurately document all nursing actions, communicate with family, case manager, and physicians, and document progress, outcomes, and changes in the client’s condition
• Coordinate or order supplies, medications, and feedings to ensure adequate amounts are always on hand
• Ensure all patient needs are met in accordance with physician instructions

• LPN/LPT/LVN degree from an accredited nursing college with current unrestricted registration and license in the state of practice and residence
• Valid driver’s license, acceptable driving record, and proof of car insurance
• CPR professional certification
• 1+ year of nursing experience preferred
• Home health care experience preferred
