RN Case Manager Lewisburg

Shift: Monday – Friday;
On‑call rotation

Job Location:

Lewisburg, WV and surrounding areas

• Provides skilled nursing care, case management, and other clinical support services to patient/families in the home setting.
• Performs other related duties as required.

• Graduate of an approved nursing program.
• Strong organizational, problem‑solving, and people skills.
• Contingent upon successful completion of drug screen, background check, and other screenings as applicable.
• Exposure to infectious diseases, blood borne pathogens, and bodily fluids.
• Frequent interaction with patients, families, and interdisciplinary team members.
• Fast‑paced clinical settings with potential for high stress.
• Required use of personal protective equipment (PPE).

• Ability to lift, transfer, and reposition patients up to 50 pounds independently and over 50 pounds with assistance.
• Frequent standing and walking for extended periods (8–12 hour per shift).
• Frequent bending, stooping, kneeling, crouching, and reaching.
• Ability to push and pull equipment (e.g., wheelchairs, stretchers, and medication carts).
• Visual acuity to read medication labels, physician orders, and monitors.
• Auditory ability to assess patient needs and respond to alarms or calls for assistance.
