$2,000 Sign‑On Bonus

Responsibilities

During a visit that can last up to one hour, providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. The Matrix Comprehensive Health Assessment (CHA) helps to close multiple care gaps with a single visit.

  • Conduct adult/geriatric assessments to include medical history, diagnosis, treatment, health education, physician referrals, case management referrals, follow‑up, and clear documentation according to Matrix guidelines and protocols.
  • Administer and order point of care and lab‑based testing as needed.
  • Work with physicians, case managers, social workers, family members, key caregivers, and ancillary medical personnel as appropriate.
  • Collaborate with Primary Care Physician (PCP) on patient education and follow‑up as necessary.
  • Provide services at the member’s place of residence, which can be in their homes or Skilled Nursing Facilities.

  • Flexibility – Visits are scheduled based on your availability. We conduct assessments Sunday–Saturday from 8 am–7 pm, allowing you to work during the day, evening, and/or weekend.
  • Competitive Pay – Earn income based on the number of assessments you complete. Monthly averages of 20–100 visits can earn $1,800–$11,500, based on a per‑assessment rate of $90–$115, depending on your state of residence.

  • Comprehensive onboarding training, a preceptor, 1:1 feedback, and free access to continuing education.
  • Support for obtaining additional state licensure and credentialing in neighboring states if needed.
  • Full‑time opportunities for PRN providers who wish to transition to a full‑time role and receive a sign‑on bonus.
Qualifications
  • Master’s Degree or commensurate experience and satisfactory completion of NP licensure.
  • Current NP licensure in state of practice to include prescription authority or the ability to obtain prescriptive authority as needed.
  • AANP, AACN, or ANCC board certification in Adult, Adult Gerontology Primary Care, Adult Gerontology Acute Care, Family, Emergency, or Acute Care specialty.
  • Current BLS, ACLS, or CPR certification.
  • One year of NP experience preferred; new graduates encouraged to apply.
  • Valid state driver’s license for the state of residence.
  • Automobile insurance coverage required for the state of residence (or ability to obtain).
  • Bodily injury: $100,000 per person / $300,000 per incident.
  • Property damage: $50,000.
  • Collision and comprehensive deductibles: $1,000 or less.
  • Strong computer skills and familiarity with employee health/medical record software.
  • Excellent verbal and written communication skills.
