Job Overview

Responsible for direct and indirect nursing care of patients in the home under supervision of an RN. The LPN will implement and evaluate nursing care with RN according to standard protocols.

Minimum Qualifications
  • Current Licensed Practical Nurse license issued by the state where services will be provided, or current multi‑state LPN license through the eNLC.
  • Valid driver’s license.
  • CPR certification within 30 days of hire date.
Preferred Qualifications
  • One year of Home Care Nursing experience preferred.
Core Duties and Responsibilities
  • Performs and monitors basic patient care according to policies and procedures.
  • Uses nursing process (assessment, planning, implementation, evaluation) to meet patients’ physical and psychological needs.
  • Provides a safe and comfortable home environment.
  • Ensures patient privacy and dignity during all care.
  • Completes assignments in a timely manner.
  • Communicates patient care problems or potential problems to the primary RN, administrative staff, or other interdisciplinary team members as needed.
  • Maintains patient confidentiality according to policies.
  • Functions effectively as an LPN in organization, communication, and prioritization.
  • Completes and documents full system assessment, including physical, psychosocial aspects, and age‑specific criteria at admission and each visit.
  • Applies nursing knowledge to take appropriate action when patient condition changes (collaborates with RN).
  • Develops and updates problem list and care plan with RN under physician orders.
  • Reports significant observed changes to RN, administrative staff, or physician.
  • Follows and updates plan of care based on assessed findings.
  • Documents patient status, interventions, responses, and complications in compliance with RN supervision.
  • Performs CPR when directed by patient’s wishes or appropriate representative.
  • Demonstrates bag‑technique per policy.
  • Observes skin condition, venous condition, extremity circulation, mobility, infiltration, phlebitis, infections, circulatory overload, abnormal signs, allergic reactions, and reports findings to RN or staff.
  • Perform the following procedures according to standards: vital signs, patient weights, specimen collection, ostomy care, urethral catheterization, pressure wound care, dressing changes, oral suction, ROM, bladder and bowel training, patient ambulation/transfer and positioning, pacemaker checks, diabetes care.
  • Performs and monitors patient teaching, adapting plan as needed and evaluating effectiveness regularly.
  • Coordinates medication administration as ordered, reporting discrepancies to RN and staff.
  • Collaborates with RN to facilitate changes in level of care and reports such changes.
  • Understands stages of death and dying as it relates to home health patients and understands nursing care needed for terminally ill or acutely ill patients.
  • Performs post‑mortem care as needed.
  • Technical duties: IV therapy (ports, PICCs, TLC catheters, sterile dressing changes, antibiotics – no PPIs, TPN, chemo infusions – no pain pump titration, cannot pull PICC), wound vacs, sterile wound care, Pleurx drain care, ostomy care, trach care, G‑tube care, Foley and suprapubic caths (cannot remove OnQ pain pump), peripheral and central labs, revolution device changes, complete head‑to‑toe assessments.
Physical Requirements
  • Lifts, moves, and positions patients of all weights with varying assistance routinely.
  • Hearing within normal range for assessing breath sounds, bowel sounds, pulse, monitors.
  • Visual acuity within normal range.
  • Exert 20 to 50 pounds of force occasionally; 10 to 25 pounds frequently; up to 10 pounds constantly to move objects.
  • Operates motor vehicle safely.
Working Environment
  • Exposure to bodily fluids, human tissues, contagious diseases, sharps, and explosive gases.
  • Exposure to toxic gases, fumes, odors.
  • Exposure to high stress and constant interruptions.
  • Exposure to electrical current and radiation from X‑rays.
  • Adverse road conditions and hazardous roadways.
  • Requires extensive travel and may encounter unpredictable or adverse home environments.
  • Work performed primarily in patient homes.
