Location: Salem
Matrix Medical Network seeks a Nurse Practitioner to perform comprehensive in-home health assessments for patients across the nation. Each visit can last up to one hour and includes review of medical history, medications, and risks to capture health insights not easily observed in clinics.
The CHA program aims to improve care quality and close care gaps during a single visit. You will assess adults and geriatrics, document findings, and coordinate with PCPs and care teams while visiting patients
