Clinic Nurse Practitioner

About the Role

We are seeking a full‑time (32‑40 hours per week) Nurse Practitioner for our Hilltop Center located in Hilltop, WV. The Advanced Practice Provider reports directly to the Director of Clinical Practice and/or the Area Clinical Practice Manager and collaborates on‑site with the Center Leadership Team.

• Provide primary and urgent care services to nursing center residents in collaboration with other medical providers.
• Collaborate with the Medical Director and/or Attending Physician for patient‑specific medical care.
• Work collaboratively with Center Leadership to achieve Align Med Partners’ mission of improving the lives we touch through high‑quality health care and everyday compassion.

• Graduate of an accredited School of Nursing program.
• Licensed Registered Nurse.
• Certified Nurse Practitioner with a Master’s Degree in Nursing from an accredited Nurse Practitioner Program, with a concentration and specialty track of Older Adult and/or Family Nurse Practitioner (FNP).
• Minimum of two years experience working with adult or geriatric population (preferred).
• APRN with an unrestricted professional license and DEA for each state in which they practice.
