Center Well Home Health is seeking a dedicated Home Health LPN to join our patient-focused care team. You will provide skilled nursing in patients' homes under the direction of an RN, implement individualized care plans, and educate patients and families on disease management and self-care.
You'll maintain accurate visit documentation, coordinate care with physicians and the clinical team, and participate in discharge planning to support recovery and independence.
