Licensed Practical Nurse

Responsibilities

• Provide direct nursing care in home and community-based settings
• Administer medications and treatments per provider orders
• Monitor vital signs and patient conditions, documenting accurately
• Support and follow individualized care and treatment plans
• Collaborate with RNs, physicians, therapists, and caregivers
• Educate individuals and families on health and wellness practices
• Ensure compliance with safety, infection control, and regulatory standards
• Respond to changes in condition and report promptly to supervising clinicians
