LVN ​/ LPN – LTC​/SNF

Job Summary:

Provide quality nursing care to patients in a long-term care or skilled nursing facility setting. Ensure patient safety, comfort, and well-being through skilled clinical interventions and compassionate support.

Responsibilities:

• Administer prescribed medications and treatments accurately in accordance with facility protocols.
• Monitor patient vital signs and observe for any changes in condition.
• Document patient care activities thoroughly and in a timely manner.
• Collaborate with interdisciplinary team to develop and update care plans.
• Educate patients and their families on health management and post-discharge care.

Qualifications:

• Valid LVN or LPN license in the state of practice.
• Proven experience working in LTC or SNF environments.
• Ability to wear a photo  and adhere to facility security protocols.
• Must have completed the COVID-19 vaccination series or possess a valid medical/religious exemption.
• Strong clinical assessment and documentation skills.
