JobStatement/Summary
The Advanced-Practice Provider delivers primary and preventive healthcare services in an outpatient Federally Qualified Health Center (FQHC) setting. This role supports access to high-quality, patient-centered care for underserved and vulnerable populations. Working collaboratively with the healthcare team, the provider addresses a wide range of acute and chronic medical conditions in accordance with established clinical protocols, state and federal regulations, and SWVHS standards.
ESSENTIALFUNCTIONS & RESPONSIBILITIES OF THE POSITION
- Provides comprehensive assessment, diagnosis, treatment, and followup care for patients of all ages.
- Conducts physical examinations and obtains complete medical histories.
- Orders and interprets diagnostic tests such as labs, xrays, and EKGs.
- Prescribes medications and therapeutic interventions in accordance with licensing scope and clinical guidelines.
- Ensures appropriate documentation in the electronic health record (EHR) in a timely and accurate manner.
- Educates patients and families about health conditions, medications, treatment plans, and preventive care strategies.
- Develops individualized care plans that emphasize wellness, disease prevention, and chronic disease management.
- Collaborates with physicians, nurses, behavioral health staff, care coordinators, and external partners to deliver integrated care.
- Responds to urgent medical situations and facilitates referrals to emergency care or specialty providers when necessary.
- Participates in quality improvement activities and complies with performance metrics and reporting standards.
- Maintains compliance with all FQHC, federal, and state regulatory requirements.
- Participates in oncall rotations, staff meetings, and clinical case reviews as needed.
- Contributes to health education and outreach initiatives to improve community wellness.
- Upholds confidentiality and ethical standards in all aspects of patient care and organizational operations.
- Performs other duties as assigned.
- Demonstrates clinical competence, sound judgment, and professionalism.
- Communicates effectively with patients, families, and team members.
- Exhibits cultural humility and patient centered care practices.
- Maintains composure in fastpaced or high pressure environments.
- Collaborates effectively as part of an interdisciplinary healthcare team.
- Master's degree in Nursing or Physician Assistant Studies from an accredited program.
- Current and unrestricted license to practice as an Advanced Practice Registered Nurse (APRN) or Physician Assistant (PA) in West Virginia.
- National certification (e.g., AANP, ANCC, NCCPA) in applicable specialty.
- DEA registration and current BLS certification required; ACLS preferred.
- Minimum of one (1) year of clinical experience in primary care preferred.
- Experience working in underserved, rural, or FQHC settings strongly preferred.
